The following must be received in our office prior to the commencement of projects or other contracted work performed for the Trustees, or on Trustees-owned properties:

• W-9: “Request for Taxpayer Identification Number and Certification”

• COI: “Certificate of Liability Insurance”

The COI certificate must show the following:

• Commercial General Liability

• Automobile Liability

• Umbrella Liability

• Workers Compensation and Employers’ coverage​
